By agreeing to participate in the NFL Alumni_Ronald McDonald House of Charlotte Invitational Charity Golf Classic ...

I give permission to the tournament, Navillus Sports Marketing Group, Inc. (hereinafter NSMG), Navillus Sports & Event Management Inc., (hereinafter NSEM), their clientele and associates of NSMG, and NSEM, to use my name and photographic likeness in all forms and media for advertising, trade, and any other lawful purposes of which is hereby acknowledged;

I agree to be photographed, recorded and videotaped by tournament, both NSMG, NSEM, their clientele and associates of NSMG, and NSEM, invited and non invited guests of clientele and its agents (NSMG / NSEM) in connection with my participation in the NFL Alumni_Ronald McDonald House of Charlotte Invitational Charity Golf Classic (event) on scheduled for May 17-18, 2010.

I agree to indemnify and hold harmless NFL Alumni, Inc., NSMG, NSEM, NFL Alumni-Ronald McDonald House of Charlotte Invitational Charity Golf Classic, Ronald McDonald House of Charlotte, all tournament related facilities, tournament sponsors, vendors, participating players and all agents thereof for any claims or lawsuits brought against such persons and/or entities and their employees and agents for such personal injury or property damage.

Participant expressly recognizes the TOURNAMENT is subject to being shortened or cancelled due to weather conditions, an Act of God, State of War, union strike or any other condition(s) beyond the reasonable control of NSMG, NSEM & Tournament. Should this occur, the special events prior to the golf portion of the Tournament will be held and all participants shall receive their gift packages. Should tournament be canceled or shortened due to unforeseen, circumstances (mentioned above), participant is not entitled to any refunds or return of monies in such an event.
